NLV Health Insurance FAQs

The questions Non-Lucrative Visa applicants ask us most — on requirements, the certificate, no-copay cover, timing, travel vs residence insurance, and cover for retirees, couples and families.

The quick answer that covers most questions

Most applicants need Spanish private medical insurance that is NLV-compliant: comprehensive, with no copay, no deductible and no waiting periods where required, valid for the required period, from an insurer authorised to operate in Spain, and documented with a certificate and proof of payment. Requirements can vary slightly by consulate, so always check the latest instructions for where you are applying.

NLV health insurance basics

Do I need health insurance for the Spain Non-Lucrative Visa?

Almost certainly, yes. If you are applying from outside the EU, Spanish consulates commonly require proof of private (or public) health insurance arranged with an insurer authorised to operate in Spain, usually with no copayments, no deductibles and no waiting periods, valid for the visa period. Requirements can vary slightly by consulate. See the full requirements.

What is NLV health insurance?

It is private medical insurance suitable for a Spain Non-Lucrative Visa application: comprehensive residence cover, from an insurer authorised in Spain, without copayments, deductibles or waiting periods where required, and documented with a certificate for the consulate appointment. Read the full guide to NLV health insurance.

Who needs NLV health insurance?

Non-EU applicants for Spain's Non-Lucrative Visa, whether applying alone, as a couple or as a family, and at any age. It is especially common among retirees. Each applicant normally needs their own suitable cover and certificate.

Can you guarantee my visa will be approved?

No, and you should be cautious of anyone who claims they can. Final visa decisions are made by the Spanish authorities. We help arrange cover designed to meet the usual NLV requirements and prepare the documentation, but final acceptance always depends on the Spanish authorities and the relevant consulate or visa centre.

Requirements

What are the NLV health insurance requirements?

Private medical cover from an insurer authorised to operate in Spain, comprehensive and comparable to the public health system, with no copayments, no deductible and no waiting periods where required, valid for the visa period (usually one year), and documented with a certificate and proof of payment. Some consulates also specify no coverage limit. See the requirements hub.

Does NLV insurance have to be from a Spanish insurer authorised in Spain?

Consulates commonly require an insurer authorised to operate in Spain, regulated by the DGSFP. That is not always a Spanish company, but a domestic Spanish policy meets it straightforwardly, which is why it is usually the cleanest route. See insurer authorised in Spain.

Does NLV insurance need to cover one year?

For the first application, cover is normally expected to be valid for one year, with a start date that fits your appointment and move. You will usually need to show suitable cover again when you renew your residence. See one-year policy.

Why do health insurance policies get rejected for the NLV?

Common reasons are copayments, a deductible or waiting periods; travel or short-trip insurance instead of residence cover; an insurer not clearly authorised in Spain; or a certificate that does not clearly meet the requirements. Most are avoidable with a policy designed for visa applications and a correctly worded certificate.

Certificate and proof of payment

What should my NLV health insurance certificate say?

It should name each insured applicant, show the cover dates (usually one year), confirm comprehensive cover from an insurer authorised in Spain, and state clearly that there are no copayments, no deductible and no waiting periods where required. It is often provided in Spanish and accompanied by proof of payment. See the certificate page.

Does the certificate need to be in Spanish?

Certificates are commonly provided in Spanish, or in Spanish and English, because the consulate reviews them. A Spanish-language certificate from a Spanish insurer is usually the most straightforward for the appointment.

Do I need proof of payment as well as the certificate?

Often, yes. The certificate shows the cover exists; the receipt shows it is paid and active. Many consulates want to see both, so keep them together and matching each other. See proof of payment.

Do couples and families each need a certificate?

Usually each applicant needs to be covered and to appear on the documentation, which normally means a certificate naming every applicant, or a certificate per person, depending on how the policy is arranged.

No copay, no deductible and no waiting periods

What does no copay mean for NLV insurance?

A copayment (copago) is a fee you pay each time you use the cover. No copay, or sin copago, means there is no such fee. For the NLV, cover is usually expected to be no-copayment, because it more closely resembles the public health system. See no-copay cover.

What is sin franquicia (no deductible or excess)?

A deductible or excess (franquicia) is an amount you pay before the insurer pays. Sin franquicia means there is none, so eligible costs are met in full. UK applicants say excess, US applicants say deductible; for the NLV you generally want neither. See no deductible.

What are carencias (waiting periods)?

Carencias are waiting periods during which certain treatments, such as surgery or diagnostics, are not yet covered. Sin carencias means cover is effective from the start. The NLV commonly expects no waiting periods where required, so the cover is real, day-one cover. See no waiting periods.

How do I check my Spanish policy for copay, excess or waiting periods?

Search the policy conditions for copago, franquicia and carencia. If any applies to your cover, the policy has that feature. What you want to see is sin copago, sin franquicia and sin carencias. If unsure, ask us to check the Spanish wording.

Full cover and public-healthcare equivalence

What does full cover mean for the NLV?

Comprehensive cover across the main areas: primary care, specialists, hospitalisation, surgery, diagnostics and emergencies, not outpatient-only or emergency-only. It should be residence cover, broadly comparable to the Spanish public health system. See full cover.

Does NLV insurance need no coverage limit?

Some consulates specifically require no coverage limit, meaning no maximum annual payout. Where that applies, an unlimited or very high-limit policy is the safe choice. Even where it is not stated, a low annual cap sits awkwardly against the public-healthcare standard.

What does "equivalent to Spanish public healthcare" mean?

Cover that lets you access healthcare broadly the way a resident does: across the main areas of care, without per-visit charges, without an amount to pay first, without waiting periods and without a low cap. In practice that is comprehensive cover with no copay, no deductible, no waiting periods and no limit where required. See equivalent to public healthcare.

Travel, international and existing insurance

Can I use travel insurance for the Spain NLV?

Usually, no. Travel insurance is for temporary trips and emergencies, whereas the NLV requires residence-style private medical cover comparable to the public health system. A travel policy is usually not accepted, so arrange residence cover instead. See travel vs NLV insurance.

Are EHIC or GHIC accepted for the NLV?

No. The EHIC and GHIC are for necessary treatment during temporary stays, not residence. They are not treated as suitable health insurance for the Non-Lucrative Visa.

Can I use my international or expat policy for the NLV?

Sometimes, but it is a frequent reason for documents to be questioned. Even comprehensive global cover must come from an insurer authorised in Spain, meet the no copay, no deductible and no waiting periods standard, and produce an acceptable certificate and proof of payment. If any is uncertain, a Spanish policy is lower risk. See international vs NLV insurance.

Can I use my existing health insurance for the NLV?

It depends on the policy. It must be from an insurer authorised in Spain, be comprehensive with no copay, no deductible and no waiting periods where required, and produce a suitable certificate. Send it to us and we will assess it honestly before your appointment.

Have the wrong policy — or not sure?

We can review a policy you already hold and tell you honestly whether it is suitable for the Non-Lucrative Visa, or arrange cover that is built to meet the requirements.

Timing, start dates and one-year policies

When should I buy NLV health insurance?

Earlier than most people expect. Request a quote early, even while preparing other documents, but choose a start date aligned with your appointment and move. Leaving it to the appointment week is the more common mistake, as underwriting, payment and documents take time. See when to buy.

Can I buy NLV insurance before visa approval or before moving?

Yes. The insurance is a requirement of the application, so it is normally arranged before approval, and often before moving to Spain, so the certificate is ready for the appointment. Whether a refund is possible if the visa is refused depends on the insurer and conditions.

Can I get cover without an NIE or Spanish bank account?

In many cases, yes, often paying by card before you move to Spain. The details depend on the insurer. Tell us your situation and we will explain what is possible.

What start date should I choose for NLV insurance?

A start date aligned with your appointment and planned move, while still showing the required period of cover, usually a year. Starting too early can waste part of the year before you move; we help you set it correctly.

Urgent appointments

My NLV appointment is soon — can you still help?

Often, yes. Straightforward cases can frequently be arranged and documented quickly once we have your details and payment. Contact us straight away with your appointment date and mark it urgent. We cannot guarantee a specific timescale, but time-sensitive cases are what this service is for. See urgent cover.

Can I get a same-day NLV health insurance certificate?

Sometimes, for straightforward cases, but we cannot promise a same-day certificate as a rule. It depends on the insurer, your age, any medical questions and how quickly payment and details come through. Tell us your deadline and we will be honest about what is realistic.

Should I buy a travel policy if I am out of time?

No. Travel insurance is usually not accepted, so a last-minute travel policy can create a document problem rather than solve one. It is better to arrange suitable residence cover quickly, even at short notice.

Applicants: retirees, couples, families and pre-existing conditions

Is there an age limit for NLV health insurance?

Age affects availability, underwriting and price more than the requirements themselves. Applicants over 60, 65 and 70 should check options early. New policies can be harder to arrange at the oldest ages, so it is best to speak to us sooner rather than later. See health insurance for retirees.

Do couples and families each need cover?

Usually, yes. The requirement normally applies to each applicant, not just the main visa holder, so every person on the application typically needs suitable cover and their own certificate, ideally with matching start dates. See couples and families.

Can I get NLV cover with a pre-existing condition?

Often, but it depends on the condition, the insurer and the policy. Spanish insurers usually ask some medical questions, and the handling varies. Raise it early so your cover and certificate are right. We can talk it through with you. See pre-existing conditions.

Can I get a refund if my visa is refused?

Whether a refund or cancellation is possible, and on what terms, depends on the insurer, the start date, whether the policy has been used, and the written conditions. We explain the relevant terms for the cover you are considering before you commit.
